These forms are provided by Paubox, a HIPAA-compliant service that has signed a business associate agreement with our practice, and your submissions are encrypted. Email sent to info@subtelnyfacejaw.com is also encrypted through Paubox. We use this information only to respond to you, schedule your care, and provide treatment.
- Information Collected Automatically
- Like most websites, our hosting provider records basic technical information, such as your IP address and browser type, to keep the site secure and working. We may use privacy-focused analytics that count visits without cookies and without identifying you.
