The Referral Promise
- Contact Within One Business Day
- We call your patient to schedule and confirm to your office that we have.

How to refer
Sending a Referral
Refer whichever way is easiest for your office: through our secure online referral form, or by email. Both are encrypted and HIPAA-compliant. All we need is the patient’s name, contact information, and the reason for referral. If you have recent records, test results, or the patient’s insurance information, please feel free to include them. For a recent nerve injury, please also call.
Helpful if you have them
- Recent panoramic and cephalometric radiographs
- Intraoral scans and your treatment objectives
- Sleep study results, for sleep apnea referrals
- Date and details of the procedure, for nerve injuries
